Reports to: Director/Program Manager
Knowledge and Abilities:
- General knowledge of Human Services
- Self-motivation and time management skills
- Ability to communicate effectively and in a professional manner with consumers, co-workers, employers and interagency staff
- Ability to assess the needs and strengths of individuals with disabilities
- Knowledge of Supported Employment and community resources
- Ability to work independently at job sites, consumer homes and community settings
- Ability to work in a team environment
- Ability to lift 50 pounds repetitively
Education and Experience:
- High school diploma
- Associate degree in relevant Human Services field preferred
- Experience working with individuals with disabilities preferred
Other:
- Possess valid Wisconsin driver’s license
- Access to a dependable vehicle for transportation of consumers
- Automobile insurance which meets minimum requirements of MTILP, Inc.
Duties and Responsibilities:
- Provide on-site instruction and supervision to consumers supported at job sites and daily living skills training/supports in consumer’s home or in the community
- Provide personal cares as needed to include toileting and feeding
- Maintain regular communication with SWO staff regarding any and all pertinent consumer information
- Transport consumers in personal car as needed
- Maintain any and all written records as required by MTILP, Inc.
- Participate in team meetings
- Promote the health, safety and welfare of consumers
- Adhere to policies and procedures established by MTILP, Inc.
