Reports to: Director/Program Manager

Knowledge and Abilities:

  • General knowledge of Human Services
  • Self-motivation and time management skills
  • Ability to communicate effectively and in a professional manner with consumers, co-workers, employers and interagency staff
  • Ability to assess the needs and strengths of individuals with disabilities
  • Knowledge of Supported Employment and community resources
  • Ability to work independently at job sites, consumer homes and community settings
  • Ability to work in a team environment
  • Ability to lift 50 pounds repetitively

Education and Experience:

  • High school diploma
  • Associate degree in relevant Human Services field preferred
  • Experience working with individuals with disabilities preferred

Other:

  • Possess valid Wisconsin driver’s license
  • Access to a dependable vehicle for transportation of consumers
  • Automobile insurance which meets minimum requirements of MTILP, Inc.

Duties and Responsibilities:

  • Provide on-site instruction and supervision to consumers supported at job sites and daily living skills training/supports in consumer’s home or in the community
  • Provide personal cares as needed to include toileting and feeding
  • Maintain regular communication with SWO staff regarding any and all pertinent consumer information
  • Transport consumers in personal car as needed
  • Maintain any and all written records as required by MTILP, Inc.
  • Participate in team meetings
  • Promote the health, safety and welfare of consumers
  • Adhere to policies and procedures established by MTILP, Inc.
  • other reasonably related duties as assigned
